Lap appendix

Staple fodder of CEPOD. This recipe is for a straightforward lap appendix.

Pre-op

  • Paracetamol
  • NSAID

Peri-op

  • Any cannula for induction, upgraded to 16G asleep
  • Modified RSI if indicated
  • Fentanyl ~200mcg at induction, total ~300mcg by end
  • Propofol ~150mg
  • Rocuronium 1mg/kg
  • ETT
  • Sevoflurane maintenance
  • 2-3g Magnesium into the IV fluids
  • Anti-emetics
    • Dexamethasone 6.6mg
    • Ondansetron 4mg towards end
  • Local to port sites
  • Reversal

Post-op

  • Paracetamol
  • NSAID
  • Fentanyl 0-200 mcg in recovery
  • Oramorph 10-30mg PO PRN 2hrly
  • PRN anti-emetics

Alternatives/ advanced techniques

  • TIVA with propofol and remifentanil, using a 3 way tap on the propofol pump to divert initial bolus into a 20ml syringe. This allows fast “RSI” hand bolus of propofol whilst keeping all delivered propofol “in the model” on the pump. Once settled, remove 3 way tap and 20ml syringe to remove the extra accidental disconnection points.
  • 2mg Alfentanil at induction, 150mcg fentanyl before KTS and 10mg morphineas appendix coming out
