Pre-op
- Paracetamol

Peri-op
- Any cannula for induction, upgraded to 16G asleep
- Modified RSI if indicated
- Fentanyl ~200mcg at induction, total ~300mcg by end
- Propofol ~150mg
- Rocuronium 1mg/kg
- ETT
- Sevoflurane maintenance
- 2-3g Magnesium into the IV fluids
- Anti-emetics
- Dexamethasone 6.6mg
- Ondansetron 4mg towards end
- Local to port sites
- Reversal
Post-op
- Paracetamol
- NSAID
- Fentanyl 0-200 mcg in recovery
- Oramorph 10-30mg PO PRN 2hrly
- PRN anti-emetics
Alternatives/ advanced techniques
- TIVA with propofol and remifentanil, using a 3 way tap on the propofol pump to divert initial bolus into a 20ml syringe. This allows fast “RSI” hand bolus of propofol whilst keeping all delivered propofol “in the model” on the pump. Once settled, remove 3 way tap and 20ml syringe to remove the extra accidental disconnection points.
