Microvascular decompression

Distant airway with the head in pins. Lasts 2-3 hours. Park bench positioning means some consideration needs to be given to lines and monitoring

Pre-op

  • Paracetamol

Peri-op

  • 18G cannula on same side as surgery (so it is facing up once patient is positioned)
  • Propofol/ remifentanil TIVA
  • Rocuronium 0.5mg/kg
  • Armored ETT, taped
  • Depth of anaesthesia monitoring, placed to avoid pin sites
  • Best to induce on trolley, then rotate trolley, slide across and position into park bench
  • Fentanyl ~250mcg total
  • Dexamethasone 6.6mg
  • Ondansetron 4mg

Post-op

  • Fentanyl 0-200mcg in recovery
  • Regular paracetamol
  • PRN oral morphine
  • PRN anti-emetics

Twists

  • Midazolam 2mg once cannula is in – anaesthetic sparing
  • Use fentanyl for induction instead of remifentanil – idea that it induces less hypotension, which is difficult to manage when you are on the other side of the room positioning the patient.
