Distant airway with the head in pins. Lasts 2-3 hours. Park bench positioning means some consideration needs to be given to lines and monitoring
Pre-op
- Paracetamol
Peri-op
- 18G cannula on same side as surgery (so it is facing up once patient is positioned)
- Propofol/ remifentanil TIVA
- Rocuronium 0.5mg/kg
- Armored ETT, taped
- Depth of anaesthesia monitoring, placed to avoid pin sites
- Best to induce on trolley, then rotate trolley, slide across and position into park bench
- Fentanyl ~250mcg total
- Dexamethasone 6.6mg
- Ondansetron 4mg
Post-op
- Fentanyl 0-200mcg in recovery
- Regular paracetamol
- PRN oral morphine
- PRN anti-emetics
Twists
- Midazolam 2mg once cannula is in – anaesthetic sparing
- Use fentanyl for induction instead of remifentanil – idea that it induces less hypotension, which is difficult to manage when you are on the other side of the room positioning the patient.
